Kernel panic via failure-path double free

Double free in the qla2xxx SCSI driver in Linux Kernel 5.7 through 6.19.2 allows local users to trigger a kernel panic. Several qla_bsg.c failure paths call bsg_done() even though completion handling already releases the request, causing the same object to be freed twice. Reachability requires local access to a system using the qla2xxx driver and an affected BSG request failure; the demonstrated consequence is a kernel crash.
