System crash via unreleased qla2xxx request object

Resource leakage in the Linux kernel qla2xxx SCSI driver allows local users to crash the system during repeated driver unloads. In drivers/scsi/qla2xxx/qla_gs.c, an error path fails to free a request object, leaving qla2xxx_srbs populated when module shutdown destroys its slab cache. The observed trigger requires repeated local qla2xxx unloading; published evidence establishes no remote path or data compromise.
