Kernel crash via concurrent USB request removal

Use-after-free in the Linux DWC3 USB gadget driver allows local users to crash the kernel through concurrent request removal. Unsynchronized dwc3_remove_requests() paths can process an out request after endpoint teardown has completed and freed it. Triggering the race requires USB reset handling to overlap endpoint teardown during adb root, and the documented consequence is a kernel crash.
