Cross-user token disclosure via incomplete cache key

Incorrect authorization in Auth0 Next.js SDK 4.11.0, 4.11.1, and 4.12.0 allows remote authenticated users to receive another user's token. TokenRequestCache keys in-flight access-token requests only by audience and scope, omitting request and session identity and reusing the first matching promise. A singleton Auth0Client and simultaneous requests for identical audience and scope are required; exposure can disclose a token and apply its permissions to the wrong session.
