Arbitrary file access via unrestricted workspace paths

Path traversal in Windsurf 1.12.12 and earlier allows remote attackers to read and write arbitrary local files through its file tools. Cascade accepts prompt-supplied workspace paths and passes out-of-project paths to its read and write_to_file tools without enforcing the project boundary. Processing an attacker-controlled project README is sufficient to access files with the IDE user's privileges even when Auto Execution is off and write_to_file is denied.
