Unauthenticated stored XSS via raw database headers

Stored XSS in Themefic Ultra Addons for Contact Form 7 3.5.11 through 3.5.19 allows remote attackers to execute scripts via form field names. The Database module stores unsanitized field names, ajax_get_table_data returns them as JSON headers, and DataTables inserts them into the DOM without HTML encoding. Exploitation requires the Database module, a crafted unauthenticated form submission, and a user opening the saved-entry view; the script then runs in that user's WordPress session.
