PCI device memory disclosure via stale BAR permissions

Improper permission revocation in Xen 4.0 and newer allows guest OS users to read memory from detached PCI devices. The libxl detach path leaves access permissions for 64-bit memory BARs in place after the corresponding passthrough device is removed. A PV guest with a hot-unplugged PCI passthrough device can map the stale BAR directly; an HVM guest also requires a compromised device model or stubdomain.
