Cross-domain memory disclosure via concurrent TSC writes

Race condition in Xen 4.13 and later allows guest OS users to expose cross-domain memory through concurrent reference TSC page writes. The HV_X64_MSR_REFERENCE_TSC handlers modify a domain-shared page without serialization, allowing one vCPU to free it while it remains in the p2m table and can be remapped into another domain. Only x86 HVM guests with the reference_tsc Viridian extension enabled can reach this path; concurrent writes can leak memory or underflow the page reference count.
