Authenticated clinical-record disclosure via encounter form reuse

Improper access control in OpenEMR before 7.0.4 allows remote authenticated users to read high-sensitivity clinical notes and care plans. The encounter forms menu remains available after the main encounter view denies access, and starting a new Clinical Notes or Care Plan form reopens the existing high-sensitivity record. Any logged-in user whose sensitivity permission is below the encounter's level can bypass the normal view restriction and read protected clinical content; no unauthenticated access is reported.
