Authenticated service interruption via closed WebSocket sends

Resource exhaustion in Open OnDemand 1.6 through 3.1.13 and 4.0.0-0.rc1 through 4.0.5 allows remote authenticated users to cause a service interrupt. In apps/shell/app.js, terminal output handlers keep calling WebSocket.send after the browser closes the socket, log every failed send to Passenger, and leave the node-pty process consuming CPU. A portal account and Shell App access are required; sustained terminal output can kill the user's PUN, fill the Passenger log disk, and disrupt the web server.
