Session hijacking via unrevoked logout tokens

Insufficient session expiration in Ash Project AshAuthentication.Phoenix before 2.10.0 allows attackers to hijack logged-out sessions. The controller's clear_session/1 path clears the Plug.Conn session but does not revoke persisted bearer or session tokens. Exploitation requires a previously compromised victim token, which remains usable until its database expiration to impersonate that user.
