Stored script execution via unescaped address fields

Stored XSS in OpenEMR before 7.0.3.4 allows remote authenticated users to execute JavaScript through patient address fields. In library/templates/address_list_form.php, address values are inserted into display elements with innerHTML without neutralizing stored markup. Patient creation and editing privileges are required, and execution occurs during address entry or when another user edits the saved record.
