Nonrecoverable fault via incoherent IOMMU dirty tracking

Improper IOMMU configuration in Linux allows local users to trigger a nonrecoverable fault by enabling dirty page tracking. The Intel VT-d capability check marks scalable-mode accessed/dirty support from SLADS alone instead of requiring both SLADS and coherent page-walk support through SMPWC. Reachability requires Intel VT-d hardware with incoherent page walks, low-privileged local access, and a workload that enables IOMMU dirty tracking.
