Protected-file disclosure via predictable generated filenames

Sensitive information exposure in Prevent Direct Access 2.8.8 and earlier allows remote attackers to read protected files by guessing filenames. The generate_unique_string function creates the secret filename component with insufficient randomness, making protected-file URLs predictable. Exploitation requires a reachable WordPress site with a protected file and enough information to determine its generated filename; successful requests disclose that file without authentication.
