Cross-request token reuse via cached request variables

Information exposure in GraphQL Mesh 0.96.5 through 0.96.8 allows remote authenticated users to reuse cached request variables. In getExecuteFn, transformedRequestCache keys an entire transformed request only by its DocumentNode, so later executions reuse the first request's variables and transformation context. Root-level or single-source transforms must be enabled, and a repeated operation can run with the first caller's token until LRU eviction.
