Restart crash via unchecked GitHub installations

Denial of service in Matrix Hookshot before 5.4.2 and 6.0.0 through 6.0.1 allows remote authenticated users to cause a crash on restart. GithubInstance processes every returned GitHub App installation through addInstallation without skipping suspended entries or containing thrown exceptions, so one failed installation aborts startup. GitHub support must be enabled, and users with manageConnections permission can increase exposure by adding organizations; the bounded consequence is interruption of the Hookshot service after a restart.
