Local system crash via NULL table cleanup

NULL pointer dereference in Linux kernel 6.11 through 6.12.12 and 6.13 through 6.13.1 allows local users to crash pSeries hosts via a VFIO group attach. After spapr_tce_set_window rejects reuse of a VFIO container across IOMMU groups, cleanup passes an unset tbl pointer to spapr_tce_unset_window. Low-privileged local access to the affected pSeries VFIO path is required; the resulting kernel oops interrupts the host service.
