Authenticated authorization bypass via OR-combined API roles

Improper authorization in tgstation-server 6.11.0 through 6.12.2 allows remote authenticated users to perform unauthorized API actions. TgsAuthorizeAttribute and TgsGraphQLAuthorizeAttribute appended the user-enabled role to action roles, causing authorization to accept either role rather than requiring both. Any enabled account can invoke most API actions without assigned permissions, but the WriteUsers right remains enforced and prevents persistent account escalation.
