Host-wide denial of service via unsafe bridge lookup

Deadlock in Xen 4.0 and later allows guest OS users to hang an entire host through certain legacy PCI device pass-through. During interrupt-remapping setup, the VT-d code looks up an upstream PCI bridge while executing in a context where the lookup's lock cannot safely be acquired. Exposure requires x86 Intel IOMMU hardware and an affected legacy PCI device assigned to an unprivileged guest.
