Remote runner crash via unchecked bitmap initialization

Null pointer dereference in Ollama 0.11.5-rc0 through 0.13.5 allows remote attackers to crash a model runner via the chat API. The multimodal path passes decoded base64 image bytes to mtmd_helper_bitmap_init_from_buf and dereferences its NULL result for invalid media. An unauthenticated crafted request causes a segmentation fault, leaving the selected model unavailable until the runner restarts.
