Reflected XSS via unescaped PHP_SELF form action

Reflected XSS in Callum Alden Starred Review 1.4.2 and earlier allows remote attackers to run scripts via crafted request paths. The plugin writes $_SERVER['PHP_SELF'] into an HTML form action without sanitizing the request-derived path or escaping it for attribute context. The attack requires no authentication, but a user must follow a crafted link; execution occurs in that user's browser under the affected site's origin.
