User-cap bypass via overbroad approval authorization

Incorrect authorization in GitLab EE allows remote authenticated users with a custom role to approve members beyond group user caps. The group-member approval API accepts a PUT request from a Guest with the Admin group member custom permission instead of restricting over-cap approvals to group owners. A configured user cap, a pending invitation, and the custom permission are required; exploitation adds members beyond the limit without data disclosure or service interruption.
