Kernel memory corruption via truncated IOMMU bitmap index

Memory corruption in affected Linux kernels allows local users to corrupt kernel memory and crash POWER pSeries LPARs through IOMMU setup. iommu_table_reserve_pages stores an unsigned-long address in an int bitmap index and compares a page-shifted DMA offset with an unshifted 64-bit MMIO address, causing wild set_bit operations. Reachability requires a POWER pSeries DDW and MMIO overlap during device initialization or local VFIO SPAPR TCE creation; the result includes out-of-bounds kernel writes, DMA and MMIO aliasing, and an LPAR crash.
