Service degradation via unreleased DMA memory

A memory leak in the qedf SCSI driver in affected Linux kernel branches allows local users to reduce kernel memory during device setup. In qedf_alloc_and_init_sb(), a failed qed_sb_init() call leaves the sb_virt coherent DMA allocation unreleased. Reachability requires the qedf driver to initialize a status block and enter that failure path; repeated failures consume kernel memory and degrade service.
