Kernel memory corruption via iommufd double release

A use-after-free in Linux 6.11 allows local users with iommufd or VFIO device access to read or write kernel memory or crash the kernel. The error path in iommufd_fault_alloc() calls fput(), whose release operation frees the fault object and context, and then decrements both references again. Access to /dev/iommu or /dev/vfio/vfio is required; triggering the path can cause kernel-memory disclosure, arbitrary writes, or a kernel panic.
