Monitoring-page HTML injection via unsafe templates

HTML injection in Vitess before 19.0.8, 20.0.0 through 20.0.3, and 21.0.0 allows attackers to inject markup into monitoring pages. The vtgate and vttablet /debug/querylogz and /debug/env handlers render query-influenced strings with text/template, which does not apply contextual HTML escaping. Attackers must influence logged query text, and an operator must view a status page for the injected HTML to affect that page's integrity.
