Kernel crash via failed hot-unplug domain attachment

Use-after-free in the Linux kernel s390 IOMMU allows local users to crash a system during surprise PCI hot-unplug. During removal, __iommu_group_set_domain_nofail() fails to attach the default domain after the platform forgets the device, leaving a NULL domain pointer that is reused after free. Triggering the flaw requires local control of PCI hot-unplug on s390 hardware; the documented consequence is a kernel crash.
