Kernel fault via stale SCSI flush request

A use-after-free in Linux and Siemens MFP firmware allows local users to trigger a kernel fault during SCSI device probing. Queue destruction leaves a freed flush request in tags->rqs[] because blk_mq_clear_flush_rq_mapping() is skipped after QUEUE_FLAG_INIT_DONE is cleared. Reachability requires local low-privileged access and SCSI queue teardown while timeout work iterates request tags; published evidence demonstrates an invalid read but no reliable privilege-escalation path.
