Memory disclosure and crash via QAT bounds error

Out-of-bounds read in the Linux kernel QAT 4xxx driver allows local users to disclose memory or crash the kernel during firmware lookup. uof_get_name() accepts an index equal to num_objs because its bounds check uses > rather than >=, then reads past the fw_objs[] array. Reachability requires local low-privileged access and a system running the Intel QAT 4xxx driver; the bounded impacts are memory disclosure and a kernel crash.
