Kernel memory disclosure and crash via uninitialized kiocb

Use of an uninitialized resource in OCFS2 in affected Linux kernel releases allows local users to read kernel memory or crash the host. ocfs2_file_read_iter() and ocfs2_file_write_iter() can receive a struct kiocb whose private field was never initialized, although ocfs2_dio_end_io() can consume that field. A local low-privileged user must exercise direct I/O on a mounted OCFS2 filesystem built with CONFIG_OCFS2_FS; the bounded effects are kernel-memory disclosure and a host crash.
