Kernel crash via invalid error-pointer dereference

NULL pointer dereference in the device-mapper core in Linux 6.9 through 6.11.7 and 6.12 release candidates allows local users to crash the kernel. When blk_alloc_disk fails, cleanup_mapped_device tests only whether md->disk is non-NULL, then dereferences the stored error pointer to clear private_data. The crash requires local low-privileged access and a blk_alloc_disk failure during device-mapper setup; no data disclosure or modification is reported.
