System crash via uninitialized command bitmask

NULL pointer dereference in the Linux kernel mlx5_core driver allows local users to crash the system during firmware health-error handling. Command initialization leaves the MANAGE_PAGES bit unset until the first such command, but mlx5_cmd_trigger_completions still processes that slot and dereferences its absent command object. The path requires an mlx5 device to report a health error before any MANAGE_PAGES command; the documented consequence is a kernel crash.
