ClusterPolicy bypass via unrestricted PolicyException namespaces

Authorization bypass in Kyverno before 1.13.0 allows remote authenticated users to override ClusterPolicies with PolicyException objects. The admission controller consumes PolicyException objects from every namespace without restricting exception lookup to a configured namespace, so an exception can suppress a named ClusterPolicy rule. Permission to create PolicyExceptions and workloads in a non-Kyverno namespace is required; a successful bypass can admit a privileged container and enable node-root access.
