Memory corruption via unchecked pixel LUT index

Out-of-bounds write in DCMTK 3.6.8 nowindow rendering allows attackers to corrupt memory via a crafted DICOM file. DiMonoOutputPixelTemplate::nowindow sizes its output from the DICOM Rows and Columns fields but uses attacker-controlled pixel values as lookup-table indexes without validating their range. Reachability requires a DCMTK application to process the file; successful triggering corrupts heap memory and terminates the process.
