Host denial of service via recursive error lock

Denial of service in Xen 4.5 and later allows guest OS users to deadlock the x86 hypervisor through an HVM or PVH virtual APIC. An illegal APIC error vector makes vlapic_error() recurse while the same state lock is still held, causing a recursive lock attempt. Only x86 hosts running HVM or PVH guests are exposed, and triggering the flaw deadlocks Xen rather than merely stopping the guest.
