Service crash via stale stream-decoder reference

Denial of service in affected Envoy 1.28–1.31 releases allows remote authenticated users to crash the proxy with crafted requests. The HTTP async client's sendLocalReply path destroys a deferred async stream before router.onDestroy() reuses its stream-decoder reference, causing a segmentation fault. Exposure requires request mirroring or ext_authz that permits Upgrade and Connection headers, and the resulting crash stops all proxy traffic.
