Unauthenticated authorization bypass via trusted proxy headers

Authorization bypass in Envoy allows remote attackers on private networks to alter trusted x-envoy request headers. When internal_address_config is empty, the default internal-address logic classifies RFC1918 sources as trusted and accepts their proxy-control headers. A default edge ingress configuration is required; injected X-Envoy-Original-Path values can bypass routing or authorization controls and expose mesh data.
