Service interruption via inconsistent connection ID cleanup

Denial of service in Quinn quinn-proto 0.11.0 through 0.11.6 allows remote attackers to crash servers via crafted QUIC traffic. Endpoint cleanup confuses the original initial destination connection ID with the post-Retry packet's destination ID, removes the wrong connection-index entry, and leaves internal state inconsistent until later packet handling panics. Exploitation requires a server that calls Endpoint::retry() and then refuses, ignores, or accepts the validated connection; the consequence is a process crash and service interruption.
