Local kernel crash via stale qla2xxx I/O entries

NULL pointer dereference in the Linux qla2xxx SCSI driver allows local users to crash the kernel during virtual-port deletion. The vport teardown path leaves aborted I/O entries stale in the outstanding-command array because it does not explicitly log out every port before deletion. Triggering the fault requires local low privilege, an active qla2xxx Fibre Channel configuration, and vport deletion; the reported impact is a kernel crash.
