Host crash via unlocked command completion race

Race condition in the Linux qla2xxx SCSI driver allows attackers to crash the host during concurrent NPIV teardown and firmware reset. The driver completes an aborting command outside its lock, allowing multiple completion paths to free and then dereference the same DMA state. Local access and low privileges are required, and the consequence is limited to host availability.
