Account takeover via spoofable PIN rate limits

Authentication bypass in Kaiten 57.131.12 and earlier allows remote attackers to gain account access by brute-forcing emailed PINs. The PIN verification endpoint trusts the X-Forwarded-For header for rate limiting, so changing the header resets the remaining-attempt counter and prevents HTTP 429 responses. Knowledge of a valid username is sufficient to submit repeated PIN guesses, and a correct guess returns an authenticated session cookie.
