Local kernel memory corruption via stale hctx reuse

Use-after-free in the Linux kernel block layer allows local users to corrupt kernel memory during hardware-queue reallocation. The blk_mq_alloc_and_init_hctx path reused an unused hardware context based on its NUMA node without verifying that its cpuhp_online and cpuhp_dead callback nodes had been removed. Low-privileged local access and a block-queue reallocation path are required; no network-facing parser is involved, and the stale callback can access freed kernel memory.
