Policy bypass via completed defrag tracker reuse

Improper defragmentation in Suricata 6.0.0 through 6.0.19 and 7.0.0 through 7.0.5 allows remote attackers to bypass policy. DefragGetTrackerFromHash checks the remove flag only for the first tracker in a hash row, so a new fragmented packet can reuse a completed tracker with the same IP ID and fail reassembly. Exploitation requires tracker collisions and two quickly arriving fragmented packets with a reused IP ID; missed reassembly lets matching traffic evade IDS or IPS inspection.
