Persistent fabric sessions via premature vport teardown

Incomplete cleanup in the Linux kernel lpfc driver allows local users to leave deleted NPIV ports logged into a Fibre Channel fabric. During vport deletion, fc_remove_host() releases the last ndlp reference before DA_ID and LOGO messages are sent, so fabric cleanup can be skipped. Exploitation requires local access and an lpfc-backed NPIV configuration; the stale fabric login can interrupt Fibre Channel availability.
