Authenticated session access via MFA code replay

Authentication replay in Mattermost 9.11.0 through 9.11.2 and 9.5.0 through 9.5.10 allows remote authenticated users to reuse an MFA code. The authentication flow accepts the same MFA code again for about 30 seconds instead of invalidating it after its first successful use. Reachability requires an MFA-enabled account and possession of a recently used code; successful reuse permits another authenticated session without directly interrupting service.
