Remote crash via dangling QUIC response encoder

Use-after-free in affected Envoy releases allows remote attackers to crash the proxy through a crafted HTTP/3 stream sequence. EnvoyQuicServerStream omits reset callbacks after RESET_STREAM without STOP_SENDING, leaving HCM's ActiveStream referencing a destroyed QUIC stream as its response encoder. An unauthenticated HTTP/3 downstream client must leave the request open, reset the stream, receive the reply, and then close the connection; the resulting stale encoder access crashes the Envoy process.
