Database metadata disclosure via unsanitized error responses

Information exposure in ZITADEL allows remote attackers to read database names, usernames, and hostnames after database connection failures. The gRPC error converter and HTTP instance and asset handlers returned raw pgx or nested parent errors to clients instead of replacing or detaching sensitive details. No authentication or user interaction is required, but disclosure occurs only when an externally reachable request encounters a database connection or instance-resolution failure.
