Privileged guest escape via XHCI stream-index overwrite

Heap out-of-bounds write in FreeBSD bhyve XHCI emulation allows guest OS users to execute code in the host bhyve process. pci_xhci_find_stream accepts a stream identifier equal to ep_MaxPStreams, then uses the out-of-range index to write guest-controlled data beyond the heap-allocated stream array. Exploitation requires privileged software in a guest configured with an emulated XHCI device; resulting code runs as the bhyve process, typically root, within its Capsicum sandbox.
