Guest memory access via stale IOMMU mappings

Improper IOMMU error handling in Xen Hypervisor on affected x86 systems allows guest OS users to access unassigned memory regions. The iommu_identity_mapping() error path fails to unwind an RMRR or Unity identity mapping after PCI attachment errors, leaving the mapping installed. Exposure requires PCI hot-attachment with xl pci-attach, an RMRR or Unity region, and a VM that continues running; service interruption, privilege escalation, and information disclosure depend on the host configuration.
