I/O hang via stalled RAID reshape

A deadlock in dm-raid456 in affected Linux kernel releases allows local users to hang I/O during concurrent RAID reshape. raid5_make_request waits for reshape progress even when dm_suspend cannot advance reshape, rather than requeuing the blocked I/O. The path requires high local privileges, concurrent I/O and reshape, plus a read-only array or a WAIT or FROZEN recovery state; the bounded result is an I/O hang.
